That periapical lesion was an accidental finding

Patient came after  consultation of implatologist with conclusion there was no possible to put an implant. Means full of motivation to save the tooth.

Preoperative x-ray and CBCT. Inflammatory apical resorption as a consequence of periapical lesion. Buccal bone fenestration in apical area. Sound cortical plate coronally.

We always can do our work in fully developed teeth and hopefully we can do it in decent way. But how can we clean  such a big space in midroot? My 80 file touched walls but how can I be sure in its work in oval canal?

the only way I know is a surgical wayCanal was cleaned and filled with BC putty at the same visit

the only way I know is a surgical way. Lingual aspect contained an old filling material ( in big diameter of oval canal). Direct vision through microscope helped to clean that area as well.
