This patient had no complaints but needed new crown and prosthetic dentist asked me to evaluate an endodontic status

Tooth was treated endodontically at least twice. The first attempt was done more than 20 years before, tooth underwent treatment with “Red Russian Paste” which caused obliteration  in root canals.  The started to hurt 12 years after the first treatment and the second attempt to re-endo was done. DB canal and palatal (partially) canals were negotiated and filled with GP.

Preoperative X-ray shows big lesion on DB root. MB root is completely calcified. No PARL on Pal root.

the decision done to go surgically and to do apico on DB root. The first exposure of root tip showed VRF (left picture), but shaving 2mm more of root I found no fracture line.
