The patient suffered from chronical pain in area of the first lower premolar. Endodontic treatment was done aprox   13 years ago (elective treatment before bridge placement).  Patient decided to change the bridge and Re-Endo procedure was done by RD. Patient has suffered for several weeks after the treatment  by finally the tooth calmed down. Patient was still feeling discomfort in the area.

On preoperative X-ray broken fragment was seen, probably RD tried to bypass the fragment but created via false leaded likely to perforation. The lingual canal was probably also blocked in attempt to negotiate The treatment plan was remove GP, locate and remove the fragment, find the true canal, repair the perf and obturate the rest. It is heard like a heroic attempt, isn’t it?

After the GP was removed the true reason was discovered. VRF has caused all the symptoms. Unfortunately the tooth should be extracted
